As a result of several field failure studies, evidence is clear that failure rates for the same product vary from site to site. The ratio ranged from 2X to 4X based on product type. The differences seem to be related to site training, site procedures, and other variables that some have called “Safety Culture.” exida defines this variable as the “Site Safety Index (SSI).” SSI is a simple way to include an impact from what many people call “systematic failures” in SIL verification. This webinar will provide an overview of the impact of SSI on random failure rates, realistic PFDavg calculations, describe the safety impact of SSI, and share an actual product example.
