Incident
The result of an initiating event that is not stopped from propagating. The incident is most basic description of an unwanted accident, and provides the least information. The term incident is simply used to convey the fact that the process has lost containment of the chemical, or other potential energy source. Thus the potential for causing damage has been released but its harmful result has not has not taken specific form.
