This webinar addresses the value and importance of investigating Safety Instrumented Systems (SIS) failures to root cause. Many companies do not understand why this is important because of either a lack of understanding, manpower issues and/or time pressure to restore the SIS.
The webinar emphasizes the fact that IEC61511 requires SIS failures to be investigated, therefore, we need to understand how the SIS has failed. For example, did it fail-safe, fail dangerously, was it a random failure or a systematic failure? Many times, SIS failures are either dismissed as being systematic or simply having the faulty equipment replaced without fully understanding why it failed and how. It should also be remembered that the probabilistic calculations performed on Safety Instrumented Functions (SIFs) are based upon random failures. If failures are not properly classified, then this will lead to lower random failures, which in turn can lead to very low and dangerously optimistic estimates on PFDavg or PFH. Consequently, this leads to overstating the risk reduction being provided by the SIF.
